Getting Started

There are only four steps to getting started …

  • Verify your insurance plan’s mental health outpatient therapy benefits and in-network, out-of-network benefits, if there’s a deductible and copay at time of service.
  • Email or call me for an intake appointment. If you are seeking to use insurance, in your email include Insurance Plan Information so I can verify your benefits. Include: Name of Plan, Subscriber Name, Subscriber Date of Birth, ID, Group ID and Provider Services Phone #¬†(located on back of your insurance card).
  • I list my session/schedule availability under my “Contact”¬†page. Depending on your availability and your desires for therapy, we may be a good match. Print/fill out/sign Intake Packet and bring into first session.
  • Attend your first appointment and bring completed Intake Packet, Insurance Card (if applicable) form of copayment or fee payment. If possible, check is preferable.
