  • I accept some insurance plans, out of network benefits and self-pay/out of pocket payments for sessions.
  • A credit card is required to be on file for no shows, late cancels
  • Co-payments, self-pay payments and deductibles are due at the time of session.
  • I accept check, cash and debit/credit card (including HSA debit cards). Receipt and or supberbill provided if requested. 
  • I am a Licensed Mental Health Provider in Oregon and in Washington

I may either be an in-network provider or an out-of-network provider on your plan. Your benefits must be verified before a session is scheduled. Should your insurance change and/or should you find out you do not have coverage-you are ultimately responsible for your bill.

Please note: I do not take OHP (Oregon Health Plan) or Medicare plans.

Late Cancel/No Show Policy

As a courtesy to yourself and myself, please arrange your day to arrive for your appointment at the scheduled time. It is my policy to bill the full fee of $150 for appointments canceled with less than 24 hours notice. If you use insurance, insurance does not pay for missed or late canceled sessions and you are responsible for the $150 fee. If cancellation occurs too frequently, therapeutic issues and fit may be discussed and re-negotiated.
